When sending in a unit for test or repair, be sure to include a copy of this SEND UNIT FORM with your contact information. Be sure to include any diagnostics or DTC error codes if available. We always recommend insuring the package for replacement value!!! AND KEEP THE TRACKING NUMBER! YOU MAY PUT ANY PERSONAL ID MARKS ON THE UNIT BEFORE SENDING IT IN.
